Competition!


We are very keen to receive feedback on our services, good or bad, as we are always seeking ways to make the experience better for patients. The service is funded by NHS Westminster for the local people of North Westminster, and as such we really want to know what local people think. In return you have the chance to win a wonderful 32GB Apple iPod Touch.


It’s simple to take part, you just need to be a resident of Westminster and come use the clinic.


After your visit, become a fan of our Facebook page          


Then, visit the ‘Review’ section of our page and post a comment to let us know what you think. Once a quarter, a winner will be selected at random form the reviews left on the page, with the first winner being selected at the beginning of October 2010.

TERMS & CONDITIONS


1. The iPod Touch competition supports the MSH@Queens Park Service and is sponsored by Metrosexual Health Limited.


2. Only valid entries will be accepted. A valid entry is from a person who places a review onto our MSH@Queens Park Facebook Page, having attended and registered with the service, and is living in Westminster.


3. Only residents of Westminster may enter the competition. Entries will NOT be accepted from residents of other boroughs or from outside London.


  1. 4.Reviews should be made using the entrants own Facebook profile/account and will not be accepted is placed using the profile/account of a third party.


5. All personal identifying information pertaining to competition entries will be stored securely in line with the Data Protection Act 1998, and destroyed once the winner is announced.


  1. 6.The current competition will operate from 01.09.2011 until 31.12.2011 (Quarter 3)


7. The competition winner will be contacted at the end of the competition period (early October) and will need to attend a small lunch time presentation to collect their prize. The winner will need to be validated to ensure (i) they are actually registered as a patient with the MSH@Queens Park service, and (ii) they are resident in Westminster (recent utility bill required as proof of address).


8. There is no cash alternative. The prize is an Apple iPod Touch 32GB.
